This is an interesting idea (and a valid area of study) however at present it reads more like an opinion piece rather than a cohesive review of ideas. I wondered if you considered framing your discussion around some pedagogical theories? The one that springs to mind immediately is social constructivism which considers knowledge acquisition as a communal event (the classic example of a 'fact' in science only being considered a 'fact' when it meets all of science's unwritten social rules matches very well with your discussion of what medicine 'expects' from its people). Theories of reflexivity may also be interesting. I also thought your paper would benefit from a discussion of hidden curricula, but that's a pet topic of mine!
